Public Records (Open Records) Request Form

  • You may request public records by phone, email, in person, or in writing. This online form will be submitted directly to the IVCCD Board Secretary, who is responsible for responding to IVCCD public record requests.

    If there is a fee for this request, you will be notified of the fee before the request is processed, and IVCCD will require receipt of your payment before the records are provided to you.
